CURRENT PHYSICAL EXAM ARE REQUIRED FOR SCHOOL ENTRY

Immunization Requirement for School Entry

Natick Public Schools, in accordance with Massachusetts Department of Public Health 105 CMR 220.000 regulations, requires a certificate of immunization that the student has been fully immunized before the student enters schools.

  • Certificate of Immunization means either:

    • a form or letter signed and dated by a healthcare provider which specifies the month and year of administration and the type/name of the vaccine(s) administered to the student; or

    • a dated report from the Massachusetts Immunization Information System which specifies the month and year of administration and the type/name of the vaccine(s) administered to the student.

Immunization requirements apply to all students including individuals from another country enrolling in NPS, as well as those attending or visiting classes and/or educational programs as part of an academic visitation or exchange program. Requirements apply to all students, even if over 18 years of age.

The purpose of school immunization requirements is to protect students and members of their community from serious vaccine-preventable diseases by ensuring high vaccination rates.

Physical Exam Requirement for School Entry

Natick Public Schools (NPS), in accordance with Massachusetts Department of Public Health 105 CMR 220.000 regulations, requires written evidence of a physical examination by a licensed healthcare provider within one year of registration in NPS or within 30 days after entry to NPS.

  • The physical exam shall contain evidence of:

    • an up-to-date immunization record.

    • vision screening.

    • written verification that the student is fit for all activities.

    • the examining healthcare provider’s written or electronic signature, and 

    • date of exam.

Physical exam requirements apply to all students including individuals from another country enrolling in NPS, as well as those attending or visiting classes and/or educational programs as part of an academic visitation or exchange program. Requirements apply to all students, even if over 18 years of age.

The purpose of physical examination requirements is to identify and take appropriate actions with respect to disabilities and medical conditions so as to enable all school children to obtain the fullest benefit of their education.
